OUR PROCESS

1. Referral and initial assessment: The process usually begins with a referral from a healthcare provider or hospital. The home health agency will then conduct an initial assessment to determine the patient's wound care needs and develop a personalized plan of care.
2. Development of a care plan: Based on the assessment, the home health agency will develop a customized care plan that may include wound cleaning, dressing changes, medication administration, and other wound care techniques. 
3. Care coordination: The home health agency will work with the patient's healthcare provider and other members of the care team to ensure that all aspects of the patient's care are coordinated and integrated.
4. Implementation of the care plan: The home health agency will send a skilled healthcare professional, such as a nurse or wound care specialist, to the patient's home to provide the wound care services. 
5. Ongoing monitoring and adjustment: The healthcare professional will continue to monitor the patient's wound and adjust the care plan as needed based on the patient's progress.
6. Patient and caregiver education: The healthcare professional will educate the patient and caregiver on wound care techniques, signs of infection, and other aspects of wound care and prevention.
7. Reporting and communication: The home health agency will provide regular updates tothe patient's healthcare provide
